简要规则描述：本项目位于上海市浦东新区云台路1800号，门诊一层大厅内靠放射科建筑面积约240平方米，施工内容新建核磁共振机房二间及设备间，控制室。项目竣工验收时，须同步提交符合国家/行业标准的第三方权威检测机构出具的效能屏蔽检测合格报告。（具体工作内容，详见招标工作量清单）

Brief Specification Description: This project is located at No. 1800 Yuntai Road, Pudong New Area, Shanghai, covering approximately 240 square meters adjacent to the Radiology Department in the outpatient first-floor lobby. The construction involves the new construction of two MRI machine rooms and an equipment room, along with a control room. Upon project completion and acceptance, a qualified performance shielding test report issued by a third-party authoritative inspection agency complying with national/industry standards must be submitted simultaneously. 合同履约期限：本项目计划工期为90日历天。具体开工日期以招标人开具的开工令为准。

The Contract Period: The planned construction period for this project is 90 calendar days. The specific commencement date shall be subject to the commencement order issued by the tenderer.

二、申请人的资格要求

2. Qualification Requirements for Suppliers

(a)满足《中华人民共和国政府采购法》第二十二条规定；

(a)Meet the provisions of Article 22 of the "Government Procurement Law of the People's Republic of China";

(b)落实政府采购政策需满足的资格要求：本项目专门面向中小微企业采购。

(c)本项目的特定资格要求： 3.1施工资质要求：具有住房城乡建设主管部门颁发的有效的建筑工程施工总承包三级及以上资质；

3.2具备有效的安全生产许可证；

3.3项目负责人资质要求：拟派的项目负责人具有建筑工程专业二级及以上注册建造师执业资格，具备有效的安全生产考核合格证书；

3.4未被“信用中国”网站（）、中国政府采购网（）、国家企业信用信息公示系统（）等官方渠道列入失信被执行人、重大税收违法案件当事人名单或政府采购严重违法失信名单且无其他不符合《中华人民共和国政府采购法》第二十二条的规定条件的情形。

3.5本项目不接受联合体投标。

(c)Specific qualification requirements for this program: 3.1 Construction Qualification Requirements: Possess a valid third level or higher qualification for general contracting of construction projects issued by the competent department of housing and urban-rural development; 3.2 Have a valid safety production license; 3.3 Qualification requirements for project leader: The proposed project leader shall have a registered construction engineer qualification of Level 2 or above in the field of construction engineering, and possess a valid safety production assessment certificate; 3.4 Not included in the official channels such as the "Credit China" website (www.creditchina. gov.cn), the China Government Procurement Network (www.ccgp. gov.cn), and the National Enterprise Credit Information Publicity System (www.gsxt. gov.cn) in the list of dishonest persons subject to enforcement, parties involved in major tax violations, or serious dishonest government procurement cases, and without any other situations that do not meet the conditions stipulate

(i)符合《中华人民共和国政府采购法》第二十二条的规定；

(i)Comply with the provisions of Article 22 of the"Government Procurement Law of the People's Republic of China";

(ii)未被“信用中国”（）、中国政府采购网（）列入失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信行为记录名单；
