二、申请人的资格要求

2. Qualification Requirements for Suppliers

(a)满足《中华人民共和国政府采购法》第二十二条规定；

(a)Meet the provisions of Article 22 of the "Government Procurement Law of the People's Republic of China";

(b)落实政府采购政策需满足的资格要求：促进中小企业发展、促进残疾人就业等；

(c)本项目的特定资格要求： 3.1本项目专门面向中小企业采购。

3.2未被列入《信用中国网站》（）失信被执行人名单、重大税收违法案件当事人名单和“中国政府采购网”（）政府采购严重违法失信行为记录名单[以查询的提交首次响应文件截止之日前3年内的信用记录为准]。

(c)Specific qualification requirements for this program: 3.1 This project is specifically designed for procurement by small and medium-sized enterprises. 3.2 Not included in the list of dishonest persons subject to enforcement on the Credit China website (www.creditchina. gov.cn), the list of parties involved in major tax violations, and the list of records of serious illegal and dishonest behavior in government procurement on the China Government Procurement Network (www.ccgp. gov.cn) [based on credit records within the past 3 years prior to the deadline for submitting the first response document].

（1）磋商代表人授权委托书原件(加盖单位公章、法人章)；

（2）磋商代表人有效身份证原件及加盖单位公章的身份证正反面复印件、近3个月社保证明或其他本单位员工证明；

（3）空白并已盖章的《磋商最终报价一览表》和《明细表》；

（4）磋商文件规定需要的提交纸质响应文件及电子文件；

（5）无线3G或4G上网卡、数字证书(CA证书)及可无线上网的笔记本一台（笔记本电脑提前确认是否浏览器设置、CA证书管理器下载等，确保和CA证书匹配可以正常登陆上海政府采购网）。

2、根据上海市财政局《关于上海市政府采购信息管理平台招投标系统正式运行的通知》（沪财采[2014]27号）的规定，本项目招投标相关活动在电子采购平台（网址：）电子招投标系统进行。供应商应根据《上海市电子政府采购管理暂行办法》等有关规定和要求执行。
